FOX News statement: Televised suicide incident result of ‘severe human error’

As I noted earlier, FOX News anchor Shepard Smith apologized profusely after airing live footage of a man shooting himself at the end of a car chase.

Executive Vice President Michael Clemente issued a statement as well:

We took every precaution to avoid any such live incident by putting the helicopter pictures on a five second delay. Unfortunately, this mistake was the result of a severe human error and we apologize for what viewers ultimately saw on the screen.
