Adding to the mounting problems facing the Phoenix Veterans Affairs health system, the Office of Inspector General is now looking into reports of sexual harassment.

As the Washington Examiner's Susan Ferrechio reported on Wednesday, an interim IG report concluded that senior VA executives in Phoenix "significantly understated" wait times and that "inappropriate scheduling practices practices are systemic" throughout the Veterans Health Administration.

The IG report also included the following revelation:

Lastly, while conducting our work at the Phoenix HCS our on-site OIG staff and OIG Hotline received numerous allegations daily of mismanagement, inappropriate hiring decisions, sexual harassment, and bullying behavior by mid- and senior-level managers at this facility. We are assessing the validity of these complaints and if true, the impact to the facility’s senior leadership’s ability to make effective improvements to patients’ access to care.

The interim report said the office would make recommendations in its final report.
